Schedule a Cognitive Assessment Visit

Is it dementia? In addition to conducting world-class research at the KU Alzheimer's Disease Research Center, the KU Memory Care Clinic offers direct patient care. To schedule a dementia evaluation, follow the process outlined here.

Schedule a Cognitive Assessment Visit

The University of Kansas Health Systems Memory Care Clinic offers direct patient care in addition to our world-class research at the KU Alzheimer's Disease Research Center.

The Memory Care Clinic provides comprehensive care for patients and their families, from early diagnosis through later stages of cognitive changes. Our neurology team will work with you to understand the cause and progression of your cognitive symptoms and help address any concerns you have.

If you or a loved one needs a cognitive assessment, here's how to schedule a visit with us.

How to Get Started

  • Get a referral from your primary care provider or local neurologist. Your primary care doctor's office or local neurologist needs to send us your clinical notes that describe cognitive changes and any abnormal cognitive test results that confirm a potential concern.
  • Provide supporting medical information. We'll also need any recent blood test results, as well as reports and images from brain scans. This information can be faxed to us at 913-945-7508.

What to Expect at Your Visit

Your cognitive assessment will include an initial visit with one of our clinicians. We tailor the testing to each individual, but it typically involves:

  • A thorough review of your medical history and records.
  • A neurological focused physical exam.
  • Cognitive and memory testing with a neuropsychologist.

Important Information

We aim to schedule your appointment as soon as possible. However, due to the high number of referrals we receive, appointments can sometimes be several months out.

If you have any questions about a Memory Care Clinic appointment, please call us at 913-588-0970.
