Cardiovascular Disease 2026 ECHO: Heart Health Powered by Team-Based Care
This series will include 4 courses on Wednesdays, beginning February 18 through March 11.
Courses will be held via Zoom from Noon – 1 PM CST.
This four-session Project ECHO learning series aims to support Kansas primary care teams in improving cardiovascular outcomes through practical, team-based approaches. This series will highlight best practices in interprofessional collaboration, social determinants of health screening and resource referral, and out-of-office monitoring strategies for hypertension and other cardiovascular conditions. Participants will learn about actionable approaches to strengthening care coordination and delivering more patient-centered, effective heart care. This ECHO is designed for primary care physicians and advanced practice clinicians, nurses, pharmacists, and community health workers.
