Verification forms can be found here.  Please make sure you are filling out all applicable forms when you have been notified that you were selected for verification

Potential forms you may need:

  • Documentation of Identity and Statement of Purpose, 2016-17
  • Verification Information Sheet, 2016-17
  • Verification Worksheet for Dependent Students, 2016-17

Summer Aid Applications

In order to receive financial aid for the Summer 2016 term, a Summer Aid Application must be submitted to our office. Please note:

    • To ensure aid disburses by the due date for summer bills, please submit your summer aid application no later than June 3, 2016
    • The last day we will accept applications to summer awarding is July 15, 2016
    • Student Financial Aid will begin notifying students about their summer awards in May
    • A valid 2016-17 FAFSA needs to be on file in order to process summer aid applications
    • The applications use the most recent version of Java script.  Java works best on Internet Explorer

  • Undergraduate Application
    • Students must be enrolled in at least 3 hours (half-time enrollment for summer) in order to qualify for most federal financial aid
  • Graduate Application
    • Students must be enrolled in at least 3 hours (half-time enrollment for summer) in order to qualify for most federal financial aid
  • Medicine Application
    • Students must be enrolled in at least 1 hour in order to qualify for federal financial aid
