Add-Drop Form for Kansas City Clinical Campus Students

Class of 2007 and Class of 2008 only

This form must be submitted at least four weeks prior to the start date of the rotation.  Only one request can be submitted per form.  A confirmation of your submission will be sent to your email address.  Your request will be processed within 48 hours and an email will be sent to you approving or disapproving your request.  Please check the clinical clerkship calendar for the beginning and ending dates of each rotation at KUMC.   Students enrolling in IDSP 900 (Special Program) must complete the online application form, obtain appropriate signatures, and submit the form to the Office of Student Affairs (3040 Murphy) four-six weeks in advance of the rotation start date.
