Mentoring is part of the Annual Faculty Assessment. The templates and resources below were created as a convenience to our faculty members in an effort to safe time with reinventing the paperwork wheel.  No item on the list is required nor are any expected to be turned in to the dean's office, but some departments may may chose to require any of them for departmental purposes.  Please feel free to use, download and customize to fit your personal needs or those of the department, division or other organizations.
