Longitudinal Elements of Fellowship

Adult Track Fellows achieve longitudinal care through their half-day Wednesday afternoon Continuity Experiences.  Every Wednesday, Adult fellows attend one of these three experiences depending on their rotation site.  They may see patients in an outpatient palliative care clinic at the University of Kansas Westwood Cancer Center or the North Kansas City Hospital Supportive Care Clinic.  If the fellow is on a Kansas City Hospice and Palliative Care rotation they will attend a home hospice interdisciplinary team (IDT) meeting functioning in the role of medical director with faculty supervision.  

Pediatric Track Fellows will routinely see patients for continuity as they transition across settings inpatient to outpatient as it is built into the PACT team's rounding workflow.  In addition they will attend the weekly CMH pediatric case conferences (IDT).  The Pediatric Fellow will have the ability to see a variety of IDT models as they move across the Adult rotation sites.
