Core Competencies

All aspects of the fellowship program operate within the framework of the common objectives of the six core competencies identified by the ACGME and ABIM, including practice-based learning, professionalism, systems-based practice, interpersonal communication skills, medical knowledge, and patient care. At the completion of training, all fellows are expected to take and pass the ABIM subspecialty exam in Endocrinology.

Patient Care
Fellows are expected to provide patient care that is compsassionate, appropriate, and effective for the promotion of health, prevention of illness, and treatment of Endocrine disease. 
The fellow will be able to:
• Obtain relevant historical subtleties, including sensitive information that informs the differential diagnosis and effectively use history and physical examination skills to minimize the need for further diagnostic testing. 
• Appropriately modify care plans based on patient's clinical course, additional data, patient preferences, and cost-effectiveness principles. 
• Independently manage patients across applicable inpatient, outpatient and ambulatory clinical settings with a broad spectrum of clinical disorders, including undifferentiated syndromes. 
• Consistently demonstrate technical skill to successfully and safely perform and interpret invasive procedures. 
• Consistently recognize appropriate patients, indications, limitations, and associated risks in utilization of non-invasive procedures and/or testing. 
• Provide consultation services for patients with basic and complex clinical problems and appropriately integrate recommendations from other consultants in order to effectively manage patient care. 

Medical Knowledge
Fellows are expected to demonstrate knowledge of established and evolving biomedical, clinical, and social sciences they apply to Endorine diseases, and apply their knowledge to patient care and the ducation of others. 
The fellow will be able to to:
• Possess the scientific, socioeconomic, and behavioral knowledge required to provide care for complex medical conditions and comprehensive preventive care.
• Demonstrate knowledge and interpretation of diagnostic testing and procedures.
• Identify areas worthy of scholarly investigation and formulate a plan under supervision of a mentor, apply basic statistical concepts and identify potential analytic methods for data or problem assessment, and effectively present at varied meetings and conferences to describe and discuss his or her own scholarly work or research.

Systems-Based Practice
Fellows must demonstrate an awareness and responsivness to the larger context and system of health care as well as the ability to call effectively on other resources in the system to provide optimal health care. 
The fellow will be able to:
• Understand the roles and responsibilities of, and effectively partner with, all members of the team while efficiently coordinating activities of other team members to optimize care.
• Identify systemic causes of medical error and navigate them to provide safe patient care.
• Consistently work to address patient-specific barriers to cost-effective care and incorporate cost-awareness principles into standard clinical judgments and decision-making.
• Appropriately utilize available resources to coordinate care and manage conflict to ensure safe and effective patient care within and across delivery systems.
• Anticipate the needs of patient, caregivers, and future care providers and take appropriate steps to address those needs.

Practice-Based Learning and Improvement
Fellows are expected to demonstrate the ability to investigate and evaluate their care of patient, to appraise and assimilate scientific evidence, and to continously improve patient care based on constant self-evaluation and life-long learning. 
The fellow will be able to:
• Regularly self-reflect upon one's practice or performance and consistently act upon those reflections to improve practice.
• Analyze own clinical performance data and actively work to improve performance.
• Actively engage in opportunities to achieve focused education and performance improvement.
• Demonstrate the ability to apply common principles and techniques of quality improvement to improve care for a panel of patients.
• Solicit feedback from all members of the interprofessional team and patients, consistently incorporate feedback, and reconcile disparate or conflicting feedback.
• Guided by the characteristics of clinical questions, efficiently search medical information resources, including decision support tools and guidelines and routinely translate new medical information needs into well-formed clinical questions.

Professionalism
Fellows are expected to demonstrate behaviors that reflect a commitment to continuous professional development, ethical practice, and understanding and sensitive to diversity, and a responsible attitude toward their patients, their profession and their society.
The fellow will be able to:
• Demonstrate empathy, compassion, and respect to patients and caregivers in all situations.
• Prioritize multiple competing demands in order to complete tasks and responsibilities in a timely and effective manner.
• Demonstrate a responsiveness to patient needs that supersedes self-interest and anticipate, advocate for, and actively work to meet the needs of patients and caregivers.
• Recognize and account for the personal characteristics and needs of each patient and modify care plan appropriately.
• Exhibit integrity, honesty, and accountability to patients, society and the profession, while actively managing challenging ethical dilemmas and conflicts of interest.

Interpersonal and Communication Skills
Fellows ar eexpected to demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationsips with patients, families, an dother members of health care teams. 
The fellow will be able to:
• Identify and incorporate patient preference in shared decision-making in complex patient care conversations and the plan of care.
• Consistently and actively engage in collaborative communication with all members of the interprofessional team to enhance patient care.
• Keep patient-specific health records organized, timely, accurate, and comprehensive while effectively communicating clinical reasoning.
• Provide effective and prompt medical information and test results/interpretations to physicians and patients.
