Prepare for your visit

In preparation for your appointment, we suggest you bring:

• Your insurance cards.
• 2 forms of identification (i.e. driver's license, social security card, utility bill).
• A list of all your current medications including strength and doses, or bring all your medicines (prescription and non-prescription) with you.
• Current immunization records and any medical records you have.
• Any medical forms that need to be filled out by your physician.
• A list of your concerns or questions, including any medicine refills you will need.

Please bring your health insurance identification card with you when you come for your appointment. You should examine your card to see if your insurance requires you to pay a "per visit co-payment" which is usually $5, $10, or $20.
Co-payments must be paid at the time of your visit, and must be paid in cash or by credit card.

Please arrive 15 minutes prior to your scheduled appointment time to complete the check-in process. To minimize your wait time, you may fill out the forms below prior to your visit.

• Authorization for the Release of Confidential Information Form
• Patient Consent to Share/Leave Medical Information Form
• Protected Health Information (PHI) Form

If you are more than 15 minutes late for your scheduled appointment you may need to wait until others are seen first. You will also be given the option of rescheduling your appointment to another time or day.
