We have options for you to refer patients with diabetes to our clinic for diabetes education, nutrition consultation, and/or diabetes medical management. See below for a description of how Diabetes Education and Diabetes Medical Management differ.
At the Cray Diabetes Center, we offer education opportunities for all patients with diabetes — whether or not they are seen for medical management of diabetes in our clinic.
If you have a patient who you see for diabetes who would benefit from education, please complete our referral form and fax to 913-588-4060 to refer a patient to our clinic.
If you are unsure what kind of education your patient needs, select the "Diabetes Self-Management Education Program" and we will contact your patient to triage him or her into the appropriate class based on the patient needs and insurance coverage.
If you believe your patient needs advanced diabetes treatment and would benefit from seeing a diabetes specialist, we would be delighted to provide diabetes medical management.
If you refer to diabetes medical management, we will adjust medication and refer to education as needed. We will continue to follow your patient for diabetes as long as they wish to attend our clinic. You will continue to be the primary care provider for all other patient needs as usual.
Complete our referral form and fax to 913-588-4060 to refer a patient to our clinic. If you would like your patient to be seen for medical management, please indicate if they need to be seen immediately, otherwise we will triage them to see the first available diabetes specialist.
For KU Providers: Referrals may be made in Epic but still must be faxed to us. You may also call us at 913-588-6022 to schedule an appointment.
