OT License or Registration: Please enclose a photocopy of your current state license to practice occupational therapy or official documentation that you are registered with the NBCOT as an occupational therapist.
Statement of Career Goals: Your statement should be approximately 400 words. Prepare a statement of your career goals that outlines: 1. Why you have chosen to pursue an OTD degree at this point in your career. 2. What you hope to learn and achieve during your study toward this degree. 3. How you plan to use this degree and what you hope to accomplish after you graduate.
Curriculum Vitae or Résumé: Highlight professional leadership contributions as well as community based volunteer leadership activities.
English Language Proficiency: All applicants whose native language is not English must demonstrate English language proficiency through either the TOEFL or IELTS testing systems. The test must have been taken within two (2) years of the first semester of enrollment. Results must be provided directly to KU by the testing service using ETS school code 6895. Photocopies of scores will not be accepted.
References: Have available the names and contact information of three references, including email address. Three letters of recommendation are required. Those who can comment meaningfully on your work in an academic or professional setting are best suited to submit letters of recommendation. However, at least one of the letters must be from a work supervisor. If you are a new graduate from and entry-level OT program, then one letter should come from a fieldwork supervisor. Each recommendation is a two-page form. Make sure they are willing to submit the reference electronically.
Transcripts: Arrange to have one official copy of each transcript from every college and university that you have attended sent directly to KU Medical Center at the following address:
