American Academy of Family Physicians Foundation (AAFP/F)

1. Grant funds will be expended exclusively for the purposes and in the manner set forth in the application.
2. Written approval for re-budgeting of grant funds must be obtained from the AAFP/F prior to any reallocation of grant funds.
3. Financial reports will be due on a schedule determined by the length of the project and a summary final accounting of the expenditure of grant funds upon completion of the project.
4. Grant funds shall be distributed on a quarterly basis.
