Goal #1

Increase the number of nurses (and other health professionals) skilled in interprofessional collaborative practice.

Goal #2

Use the collaborative practice model and the transitional care team to engage patients, their families, and communities in promoting positive health care outcomes.

Goal #3

Use outcome evaluation data to support the dissemination of best practices in inteprofessional collaborative care.

Goal #4

Establish a sustainable plan and infrastructure to leverage the Interprofessional collaborative practice project to other units throughout the KU hospital and beyond.

First Quarter and Ongoing Objective Two

Publicize the IPAC-Peds model beyond KUMC

  1. Add a nurse manager from another acute care unit in the hospital to the IPAC core team.
