Superior Supervisor Nomination Form

Having an excellent supervisor makes work great! Use this form to nominate a supervisor that deserves to be recognized for the Superior Supervisor award. Supervisor must be a current KUMC employee and have worked for the University for at least one year. Winning supervisors will receive a $500 cash award.
