The purpose of this letter is to inform you that your appointment to the position of [Employee Title], [Position Number] will not be renewed for Fiscal Year 2012-2013. This notice is provided in accordance with the Handbook for Faculty and Other Unclassified Staff*; Section III., Unclassified Professional Staff Regulations, page 130 et seq. of the Handbook for Faculty and Other Unclassified Staff. As a result, your employment with KUMC will end on June 30, 2012.
If you believe that the decision to give notice of non-reappointment constitutes a violation of established procedures of the university or of the unit, and you believe that you have been unsuccessful in obtaining a favorable reassessment of that decision through this office, you may appeal.
This letter is intended to serve as notice of a final agency action by the Medical Center. Please be advised that Dr. Barbara Atkinson, Executive Vice Chancellor of the Medical Center, is the agency officer who should receive service on behalf of the Medical Center of any subsequent petition for judicial review of this final notice.

No reason for non-reappointment is required, however a generic reason if appropriate may be inserted here. Departments should discuss with Human Resources and Legal prior to inserting language explaining the non-reappointment.
