The Privacy Rule mandates specific criteria for using protected health information for research. This site contains HIPAA-compliant forms and templates to be used for research projects at KUMC.
 

		KUMC consent forms for human research have been modified to include HIPAA authorization elements. Two templates are available: Consent Template 1, for sponsored projects and Consent Template 2 for internal studies. These templates are posted at on the Human Subjects Committee Forms page at: http://www2.kumc.edu/researchcompliance/hscforms.htm
