DIRECT SERVICEHome-based services Visits to the family's home by various team members are provided.
The frequency of visits is determined by the needs of each individual
child/family which may be weekly, alternating weekly, monthly, or quarterly.
Home visits reflect child goals and adult goals based
on the Individual Family Service Plan outcomes and questions as they
arise from the family. Community-based ServicesVisits to home, daycare provider, extended family member's home (e.g.
grandmother's), as well as other agencies involved with the child/family
are provided to the families. The frequency and length of sessions are
determined by input from the family and provider. Child and family goals
are incorporated into the community-based service.
PlaygroupThe Hartley Family Center provides a weekly playgroup (1 1/2 hours) for children and families. Activities within the playgroup follow a monthly theme based on a children's book. Communication and listening are the focus within individual and group activities. Parents are guided and supported by staff in a variety of play situations with their children and peers. Books for each theme and enriching activities are provided for families. |
