Genetic Counseling Services and Evaluations involve consultations for patients concerned about:
- a hereditary condition
in their family
- a child with a birth defect
or mental retardation
- other genetic or rare
conditions
- family cancer syndromes
- genetic testing
- other conditions and issues

- Patients may refer themselves, however, insurance providers may require a referral
- Medical records are frequently helpful to review during a genetic clinic visit, these can be requested from a primary care provider in advance, and faxed to our office, indicating your name and appointment date on the fax (fax number 913.588.4060)
- The name of established medical provider(s) can be helpful for communication with regarding issues such as followup management
- Completion of a family history form can be helpful
