The new animal user should complete the top half of the first page of the New Animal User Training Form/Checklist. The Principal Investigator should review that information, list (in the table) all procedures that the new employee will be performing, then sign and date it. PI should review (with the new animal user), the ACUP(s) and all addenda, and should supply one copy for his/her training records.
If the new animal user comes to the training without the first page of the form completed and signed, she/he will be asked to leave, and re-schedule the training date. Training needs cannot be adequately evaluated without this basic information.
After the first page of the training request form/checklist has been completed and signed, please contact Dr. Douglas Brandt (firstname.lastname@example.org) and copy Erin Taylor (email@example.com), requesting a short meeting to receive your training assignments. Bring with you to that meeting, the entire seven-page training request form/checklist and also a copy of each protocol (and any addenda) on which you will be working.
Note about the training form: Our animal user training program is evolving based on the feedback provided by KUMC research community. Therefore, the New Animal User Training Form/Checklist gets occasionally updated. Please do not save the form in your files, but rather use the most updated version that is posted on this website.
