Schedule your biometric screening and earn your HealthQuest Rewards points

In three easy steps, you can save $240 on your health plan premium for Plan Year 2017. This year's HealthQuest Rewards Program (the state of Kansas health and wellness program) began Nov. 16, 2015, and will run through Nov. 15, 2016.

Step 1: Obtain your biometric numbers in one of three ways.

  • Attend the free onsite screening events (worth five HealthQuest credits). More information on the onsite events below.
  • Schedule a preventive screening visit with your physician.
  • Order an at-home screening kit offered by HealthQuest (worth five HealthQuest credits). Read more about biometric screenings.

Step 2: Once you have your biometric numbers, you will need to complete the mandatory Health Assessment Questionnaire, which will earn you five credits. Complete the questionnaire here.

Step 3: Earn an additional 25 credits by participating in various activities ranging from challenges, virtual coaching, preventative doctor's visits and much more. Read more about the rewards program.

Log in here (or call 1-888-275-1205, option 5) to make your appointment now. and locate the BULLETIN BOARD on the lower right. The first item under the BULLETIN BOARD will be Screening Appointment. Click there and then choose the city in which you would like to attend an event. Note: your login ID is your email address.

Testing is conducted in 30 minutes or less using a simple finger stick. Results you will receive include:

  • Total cholesterol, HDL, LDL and total cholesterol-to-HDL ratio
  • Triglycerides and glucose
  • Systolic and diastolic blood pressure
  • Height, weight and body mass index (BMI)
  • Waist circumference

Remember to fast for a minimum of nine hours prior to your appointment. Please drink water to stay hydrated; black coffee and tea are also okay (no cream or sugar). Take medications as usual.

Who is eligible to participate? Benefits-eligible state and non-state employees who are enrolled in the state employee health plan or who have waived coverage in the plan, as well as retirees and spouses who are enrolled in the state employee health plan, may participate in the HealthQuest Rewards Program.
