What is the TEAM Evaluation Clinic?

This clinic provides comprehensive evaluations for children usually age four and older. It is especially suited for children with moderate to severe developmental delays or disorders, concerns in multiple areas, or with complex medical/neurological histories. Assessments, recommendations, and referrals can be made in the areas of health, motor, communication, school, daily functioning, behavior, and psychosocial/emotional development.

Interdisciplinary Team Members Are:

The team is designed for each individual child based upon his/her and their family’s unique needs. Team members can include many of the following:

  • Audiologist
  • Developmental Pediatrician
  • Psychologist
  • Nurse Practitioner
  • Nutritionist
  • Occupational Therapist
  • Parent Educator
  • Physical Therapist
  • Speech/Language Pathologist

When Is the Clinic Held, and What Happens on Clinic Day?

  • The clinic can include several appointments. If possible, they will be scheduled on the same day or on consecutive days.  
  • Please check in 15 to 30 minutes before the first appointment to room 1008.  Be sure to bring the child’s insurance or medical card and any other requested materials.
  • Participation in the evaluation by the parents or someone who is very familiar with the child is very important and will assist in final assessment and recommendations.
  • Immediately following the last appointment, all team members will meet, and this will be followed by a conference with parents to explain results and recommendations. Many families invite members of their school team for the interpretation conference. If necessary, the interpretation conference can be scheduled for another time.
  • A follow-up appointment will be discussed and can be scheduled before you leave.
  • Following the evaluation, a typed report will be mailed to the parents,  the child’s primary care physician, and to the referring physician or psychologist.

To Prepare for Clinic Day:

  • Bring someone with you to care for your other children or just for support if you would like.
  • You may want to invite the team that is involved with your child’s care or education to the evaluation or the interpretation.
  • You are encouraged to bring snacks and/or a favorite toy for your child.
  • Prepare your child in advance by telling him/her that she/he will be meeting new people who will ask him/her to play with a variety of toys and/or answer questions, but you will be there as well.
