Fellowship Electronic Application

 

Instructions for completing an electronic application

1. Click on the link below entitled 'DBP – Electronic Application'.
2. When asked if you would like to ‘open’ or ‘save’ the file click the ‘save’ button to save the application to your computer.
3. A window will appear in which you will be able to choose a location to save the file and name it.
4. Title the file as follows – Last Name, First Name – DBP Electronic Application and save the file to an appropriate location.
5. Complete the application by filling in the gray text boxes and making appropriate choices from the gray dropdown menus.
6. Check the box at the bottom above the signature box indicating
7. Save changes to the application.
8. Email the application, a current photo of yourself and current Curriculum Vitaeas attachments to swaqiah@kumc.edu
9. In addition to the above submitted electronic documents, please mail the following to address listed below
   • Three letters of recommendation (one of which should be from the director of your residency training program)
   • A transcript or report of your USMLE , any other licensing or certification examination scores
   • If you are Board Certified in Pediatrics please send documented proof of that as well
