Please address the following questions

  2. Why do you want to join the Baby Jay team?
  3. In your opinion, describe the role of the Baby Jay team at University and community events. Give at least one example of how you would fulfill the role at an event.
  4. What type of mascot, handler, or other relevant experience do you have that you feel would help you as part of the Baby Jay team? (i.e. high school mascot, leadership, tumbling, etc.)
Serving as the KUMC Mascot or handler requires responsibility, energy and effort. This includes attendance at a variety of University and other special events. Checking the box below indicates that you understand the nature of this position and you are willing and able to make the commitment to the terms of the contract stated below.

I. I certify I meet all of the following requirements:

  1. I am currently a KUMC student.
  2. I am in good standing with KUMC and not on any type of academic or disciplinary probation.
  3. I have no health or physical issues which would hamper my ability to perform as the KUMC Mascot or which might cause performing as the KUMC Mascot to be unsafe to my health.
  4. I am covered by a personal medical insurance plan and a vehicle insurance plan.

II. Furthermore, I understand:

  1. If selected, I agree to serve as the KUMC Mascot or handler for the entire academic year unless alternate arrangements are made with the Office of Alumni & Community Relations . The majority of appearances as the KUMC Mascot and handler will be during the school year, on weekends, or in the evenings.
  2. I am expected to participate in one required training sessions.
  3. I must maintain a campus email account and check it daily.
  4. I will make an effort to keep my schedule flexible and to accommodate weekend, night, and last minute requests.
  5. If for any reason I am unable to attend a function as the mascot to which I have committed, I am responsible for finding another team member to replace me.
  6. The Baby Jay Program plays an important role in the University and the community. As such, I will always know that as the mascot or handler I am an official representative of KUMC, its students, faculty, staff, and alumni. I will always conduct myself in a manner to uphold the character, tradition, integrity, and spirit of KUMC.
  7. I will adhere to all KUMC rules pertaining to Mascots.
  8. Failure to adhere to any of the items within this contract will result in the suspension or dismissal from being the KUMC Mascot.
By checking this box I will fully adhere to the contents of this application if selected to represent the University of Kansas Medical Center mascot.
