Application Procedures for the
Entry-Level Program in Occupational Therapy
NOTE TO STUDENTS:
The 2008 Application period has ended. Students planning to enter the OT program in 2009 may find our Admissions Procedures Presentation helpful.
The following steps summarize the application requirements. All forms required for application are available for download in PDF format here: 2009 MOT application packet. Individual forms may also be found below.
- Print and carefully read the OT 2009 prerequisites & procedures document.
- Print the Application Checklist to use as a guide during the application process.
- The following application materials must be sent directly to the Occupational Therapy Education department by the APPLICANT no later than Dec. 31, 2008:
a) Print and complete all fields on the School of Allied Health Undergraduate Application to apply for the Bachelor of Science in Occupational Studies. ALL APPLICANTS MUST INCLUDE A CHECK FOR $60.00 APPLICATION FEE (payable to School of Allied Health) with the completed application form.
b) Tell us a little about yourself, using the Occupational Therapy Education Application and describe completion of your Prerequiste Coursework and your plans for completing any ongoing courses.
- The following materials also must be sent directly to the Occupational Therapy Education department no later than Dec. 31, 2008, but are NOT submitted by the applicant:
a) Have a supervisor document your Work or Volunteer experiences with one or more individuals having special needs. Note that the completed form is confidential, and should be sent directly to our admissions committee by that supervisor. Make additional copies of the Confidential Volunteer/Work Verification Form as needed. In order to expedite the timely return of these forms to our office, you should provide a stamped envelope with each verification form you disperse.
b) Two sealed, Official Transcripts from each institution (all colleges, community colleges, universities) are required (One KU transcript).
c) Three Letters of Recommendation are required.

Please do NOT submit any materials or forms not specifically requested by the OT application. Please do not alter the forms in any way. Should you require additional forms, either retain a photocopy of the original forms or link to the OT Education website.
