The application forms for this program are undergoing a significant change to a new online format for students starting the program in summer 2011. Transcripts
Arrange to have one official copy of transcripts from each college and university that you have attended sent directly from their records office to the KU Department of Occupational Therapy Education. Copies are not acceptable. International transcripts require additional analysis and may require extra time to complete.

Volunteer/Work Verification
Have a supervisor document your Work or Volunteer experiences up to and including the past five (5) years. Forty (40) hours must be documented and at least 50% of the time must be direct contact with individuals with special needs. Note that the completed form is confidential, and should be sent directly to our admissions committee by that supervisor. Make additional copies of the Confidential Volunteer/Work Verification Form as needed. In order to expedite the timely return of these forms to our office, you should provide a stamped envelope with each verification form you disperse.

Recommendation Letters
Request a letter of recommendation from three (3) people. Letters may be from a supervisor, professor, pastor, coach or other adult who can attest to the applicant's potential for sucess in the program and in the profession.

International Students:
An applicant is considered an international student if he or she requires a visa, or currently resides in the U.S. with non-immigrant status, or currently resides in the U.S. while applying for permanent residency. Additional requirements and documentation, such as proof of English language proficiency, are required for international students to become eligible for KU programs. Please review the information for international students before applying.

International Transcripts: In addition to sending an official copy of each transcript, as above, have your transcripts evaluated by World Education Services OR Educational Credential Evaluators. The evaluation must include course-by-course evaluation, grade evaluation, and indicate the degree earned in U.S. terms (bachelors, masters, doctorate, etc). An official copy of the evaluation is required.

Alternatively, you may send a check made payable to KUMC for $25 (U.S.) for evaluation by the KU Graduate Application Processing Center. If this option is selected, the official transcripts must arrive at least 2 weeks prior to the admission deadline.

Additional Information for Applicants
After submitting application materials, students must meet a number of additional requirements and submit additional documentation prior to entering the program. Please review this information now so you'll be prepared. Once accepted, a position in the next class must be reserved by signing a contract of acceptance and providing a $400 non-refundable deposit. The deposit amount will be applied toward program fees upon officially entering the program.

In addition, all accepted individuals must provide proof of current certification in First Aid/CPR, current health insurance coverage and pass a criminal background check.
