- KUMC Level II Midterm Feedback Form - Feedback about on-going fieldwork experiences is sought from students mid-way through the experience in order to provide our fieldwork coordinators with information about the setting and the student's perception of their peformance. (25 questions with mixed "Yes/No" and narrative responses)
- KUMC Level II Fieldwork Survey - This survey is administered to students after each Level II Fieldwork experience, and provides feedback about academic preparation for the fieldwork, and the actual use of that information during the fieldwork. (13 multiple choice questions and 3 optional questions that allow a narrative response)
- Survey for supervisors & employers of graduates from the KU MOT program - This brief survey summarizes the views of experienced OT professionals regarding the academic and practical training provided to our students. (19 questions, multiple choice responses)
- KUMC New Grad's Perspective on Career Preparation Survey - This survey allows newly graduated students an opportunity to reflect upon our program, and to provide feedback about various aspects of our program. (20 questions, multiple choice responses)
- Collaborative Fieldwork Survey - This survey was designed to assess the fieldwork experiences of students enrolled in different Occupational Therapy education programs across the United States, particularly those in more community-based or non-traditional settings. (2-part survey with 25 questions total; mixed multiple choice and narrative responses)
These links allow Faculty and Staff to generate reports summarizing data from surveys completed on-line, and to view the completion status for each survey.
