To help each of you follow standard formats for these assignments, you may download these forms as electronic files and complete them using your own word processor. Simply follow the directions appropriate to your needs:
Disorder Summary
- PC-users click HERE using the right-hand mouse button, choose the 'Save link as ...' option and select the destination directory for the file.
- Mac-users hold down the 'option' key as you click HERE with your mouse, then select the destination directory for the file.
Please complete the summary, save it, then submit the file electronically. Please format the file only as a Rich Text Format (xxx.rtf) or as a MSWord document (xxx.doc)! Be sure 1) your name and 2) the disorder name are both included as part of each file's name! (e.g., "Radel_MS.doc", "Radel_ParkDis.rtf") If you experience problems, contact an instructor and we can work out an alternative, but do this well before the deadline!
